I also had to adjust the screw hole on the scale grip. I'm sure your cz isn't out of spec. Almost certainly the scale grip is not drilled correctly. I find the grip screw plus a small rubber O ring hold the grip on well.
I had feed problems like that with my .40 TS. A few things that can help are throat the barrel, polish the feed ramp, polish the upper part of the chamber, increased strength recoil spring (14 lb), and use Berry round nose bullets. Good luck!
Heavier bullets typically impact higher than lighter bullets at the same power factor. The heavier bullet is travelling slower, so spends more time in the barrel which may be rising ever so slightly. That may be what you're seeing.
I stopped when I had the width at .138" and I liked the sight picture. I don't remember the depth. I did it to obtain a faster sight picture for competition.
