My guess would be that it is firing out of battery. When you got the slide lightened did youalso go with a lighter recoil spring? If so, I'd try the original to see if it goes all of the way into battery before firing.

My problem was the bolts that went up and into the primer tube housing weren't straight and when tightened would tilt the assembly and bind the primer bar. I don't know if I caused that by overtightening them or if they were drilled wrong but Dillon sent me a new one and all is well. I broke one or two of those silver rods before I found that they weren't the problem.

I swapped parts with a friend until I found the one that was causing trouble for me, though it sounds like you have multiple. Do you have a friend nearby that you could borrow a primer assembly to test?

Correct on the first instance. For the second... The rules provides a squib as an example of unsafe ammunition. It is not the only type of unsafe ammunition covered by the rule. If it was determined that it was indeed a case separation, then I would consider that to be unsafe ammo and would not grant a reshoot. I would also want to make sure that the gun was still safe to use (undamaged) and that the remaining ammo did not show any visible defects before I would let the shooter continue on the next stage.
