I had an issue like this when using a heavy hammer spring on a RRA NM trigger group (on a 5.45x39 upper that needed more smack to set the primers off).
The face of the hammer that engages the disconnector and the disconnector were rough enough that if I pined the trigger and cycled the action, the hammer wouldn't come off the disco and the trigger didn't reset. If you cycled it without the trigger being pinned there was no issue. I lightly polished the engagment surfaces (without changing the geometry) and solved it for me.
Could also be a weak trigger spring, or incorrectly asembled .

Got an otis kit off a prize table. Liked it well enough to buy another. Like the guy above said it is about the size hockey puck does a good job and is convenient, what's not to like. I have several that I just leave in my various rifle bags.

I wipe off feed ramp every time I shoot. Other than that not often.
Yep, that's about what I do for my open gun. I use the swabs midway sells. I lock it open, wipe the breech face/extractor hook, the feed ramp, and add oil if it seems dry. It gets a full tear down once to twice a year, usually one practice session or club match before a major. (I never like to take a gun to a major that doesn't have at least a couple hundred rounds on it).
To be fair some of this depends on what you use for powder and your environment, also on how tight your gun is (and I believe how lightly it is sprung).
On the other hand if having a spotless gun makes you happy, then that's what you ought to do :-)

what happens if you slowly hand cycle the gun? Is the extractor to tight? Maybe a sharp edge on the bottom of the breech face?

Sounds like a extractor adjustment needs to be made.

Tight how? lots of tension sliding a round up the breech face or hard to get out of the slide? if you load a dummy round, and remove the mag and slowly hand cycle it what happens?

Think you could get a 2011 to run on this stuff?Might be an option for 3-Gunners that don't have a minimum power factor provided that it can take down steel. (Which I'm curious about the physics of)
Probably could get a 2011 to run it if you could get a barrel to use (otherwise you'd have to make one which would be a pain). If you want to shoot one remind me and I'll bring mine to the next match.

Yep.. my trig is a bit rusty but if that is linear that is like .003 to .004 degrees... not betting any thing we use is going to care about that.
