BigDave Posted April 11, 2006 Share Posted April 11, 2006 That story of the reactor accident is a pretty good lesson (yes, I have a firm grasp on the obvious, I know) on unintended consequences. I read a book a few years ago titled The Logic of Failure that detailed what happened as part of a doctoral examination of decision making that leads to such things ( and lesser things). Chilling. And, believe it or not, that crew that blew up that things were indeed experts, highly rewarded for their safety record, which the author concludes lead to their demise. Very Cool At first glance she does look crazy though The Serpent's Wall kept me reading. = her writing is very good in tieing the story to today.
