Various people sell "HatCams" rigs that attach to your head/hat/ear protection and transmit wirelessly to a reciever behind the shooter. From there, you can attach a camcorder or MP4 recorder with A/V inputs to record the video signal. I've actually made a unit like this that uses a really small, fixed-focus camera that is powered by a 9V battery. The camera is mounted on my ear protection, so it doesn't get in my way while shooting.
I also have a 2nd camcorder that I have someone film me from behind. Later I use Pinnacle Studio 11 to sync the two videos up, with the 3rd-person camcorder view as the primary screen and my head-mounted camera footage as the PnP. The outcome is very nice.
Unfortunately, I've never posted any of the video footage in order to protect the privacy of the people I shoot with.
