You need to file the back side(long side) of the follower so it doesn't catch the extension.
Don't need to touch the extension, unless there's a bur.

definitely seems to vary, between lowers.
I have both an NFA lower and a JP GMR-13
definitely depends on your whole setup as well.
I originally ran a JP upper & scs on the NFA lower. every 5 rounds I would have a FTF or FTE. switched to the JP lower and zero problems for the past 1000 rounds.
I now run a ramped taccom upper and an armaspec SRS on the NFA lower. no problems for the past 1500 rounds.
All on factory blazer ammo.
Glock factory & ETS mags.
I’d imagine hollow points and/or longer OAL’s will affect things as well

Biggest downside with the NFA/PSA lower is the slop front to rear of the mag/mag well
depending on your bolt, buffer, ammo, & barrel combo

I’m sure there’s variations between mags, followers and extensions.
I’m currently running an ETS mag with a TF +10 extension. Have about 1000 rounds through it without problems.
No mods were made to the extension, only the follower.
Reason I would advise against modifying the extensions, if it’s not needed.
the extension with oxidize if you expose the bare aluminum.
