double feed in my SP01 custom

I have gotten a couple double feeds in my SP01, what would be causing this?

two live rounds, maybe weak mag springs.

the thinner springs in the 19 rnd mag are not the strongest springs to begin with.

Sounds like a mag problem, will the top bullet stand up with minor effort (mag lips too wide). With bullets in the mag and seated in the gun ... is the top bullet touching the ejector?

I got some of those shooting 147's in my shadow till i loaded a little shorter and shaved my slidestop down.
