First, they estimate costs based on setup and programming times for CNC type tools. Second, they do favor volumes. The first part is extremely expensive, but after quantities of 20-30, the cost is more reasonable. Many of the parts I've drawn up are simple and straightforward enough that a standard machinist could make them in 30 minutes, and are well within the capabilities of a home-shop.
