45 ACP sizer die not working

After many rounds of 45 on my 650 with my RCBS sizer die it stared crunching the top of the casess, see picture.  
probally 1 every 7-10 rounds.  I have cleaned, adjusted and fiddled.  I figured it’s probally something 

I am over looking.  Any suggestions before I chuck the die and buy a new one.

The cases are not lined up with the die. That is not the die's fault. Check and see if there is media or some other crud in the shell holder that is preventing the cases from going in straight.

MY old .45 RCBS sizing die is dead flat across the bottom with no radius and very little taper into the carbide insert.

The tiniest of misalignment would keep a case from entering it on the sloppy Dillon shell plate.

You will likely have to keep a fingertip on each case and guide it into the die.

I got tired of that and went to preprocessing cases in single stage.  If I were going to load a lot on the 550 I would get a Dillon or Lee sizing die, but it has only been for about a thousand .45 Small Primer cases in the usual 9mm press and I am nearly done with those.
