I have read or Been Told, 357 Sig notorious for this. And that it was because of the very short neck case mouth

You have to be very careful of setback in a 9mm PCC, because the case is not fully supported.  I had a couple of brass blowouts.  Not fun.

 

In a fully supported chamber, setback will increase pressure, velocity and recoil.  I've never had a case separation in any of my 9mm Open guns.

I've never had a Kaboom, but I've read here many times that .40 Major

is prone to Kabooms using fast powders and heavy bullets, if setback

occurs.

That's probably not what you think it is. The bullet was pushed in too far probably from someone trying to clear it and smashed the bolt or slide into it lengthwise. When the round was fired the resulting compressed powder charge caused the casing to blow out.

this is the most likely cause .

if the round had been fired the bullet would no longer be in the case.

the bullet would still not be in the case...
