Used to be couldn't mandate a reload if movement was involved. It changed in last few years. Don't ask me when, I only pay slight attention to rule changes anymore.
Don't know if rmr match winners are hp or full metal jackets. Hp's have no exposed lead on base. fmj's have exposed lead that gets vaporized and could be what's fogging glass.
The ports are angled towards the glass??that would explain a lot. I would try reciting front of hole forward a bit and see if that helps.if not can welder replace. I would lean to replace.
Welding on one side of Barrel will deform bore to some extent. May affect accuracy. New bbl is best solution. May try to angle holes forward before welding. Can't hurt.
In my experience a lot of dot mounts the locator posts are to loose and dot moves around shakes screws loose. Pins should be almost a light press fit.a dab of epoxy with release agent on pins may help.
From the diagram it looks like 3 separate shooting areas but I also know arguing about a stage design based on a 1 dimensional drawing is a waste of time.
A true gunsmith with a mill can do it easily. Talk to fellow shooters in your area. Make sure the smith has done before, you don't want to be the experiment.

This is not a USPSA problem this is a local problem. In my area we have 25-35 yd targets in every match. Seldom have movement more than 15 yards without shooting. If your local club puts on a crappy match shoot elsewhere or step up and help run a better match.
The hammer will not fall when you pull the trigger on a flagged gun. The disconector de activates the trigger from the sear unless the bolt is fully in battery.
