650 primers upside down and sideway

Also make sure the primer ram assembly is not coming apart.  It is a pressed together unit and the ram needs to be flush with the threaded body-top.  The last time I reloaded 45 on my 650 the primers were flipping and it took awhile to figure this out. I had to put the ram unit in a vise(machinist-no teeth) with a socket or whatever fits and put it back to flush.  No problems after doing this.

Also, check shellplate rotation.  I had this problem w/ small primers on my 650 when the plate hadn't completely stopped indexing as the case came down onto the primer.  I'm running the mods (bearings, ltwt index ball, lightened ball spring) to smooth out the index and found this.
