Need help with weird malfunction

I am having a malfunction where the empty case ends up behind the ejector and the bolt jams it into the back of the ejector. When this happens I have to get a screwdriver in the ejection port to pry it off the ejector and clear it.  This happens about once every 2-300 hundred rounds. Other than that it runs great.

 

 I am pretty new to the PCC stuff and am not sure how to diagnose it.

 

The gun is  NFA lower,bolt assembly and barrel with a regular 223 upper. running 125g blue bullets over 3gr clays at 1.10

When you pull the charging handle fully to the rear, how far past the bolt catch does to travel, if it's more than about 1/2"

you need to run a spacer in the buffer tube,

ether a longer buffer, 7 quarters in the buffer tube or a spacer to reduce the bolt travel

my thinking was that 3grains of Clays and a heavy buffer/spring combo might not have enough recoil force to move the bolt all the way to the ejector, or possibly not allow the ejector to hit the ejecting brass with enough force to successfully eject it...
