GMR-13 Seperating cases

So out of the First Couple Hundered Rounds I have had half a dozen separated cases, using my own reloads. I have shot thousands of rounds of these thru my pistols and never separated a case.

Reloads= Range Brass...147 xtreme HP's over 3.3 Grains of Tight Group @ 1.130 on a 550, Dillon Carbide 9mm Die, Lee Factory Crimp set real light. I case gauge a few rounds every now and then as they come off the machine. I suppose it's likely that it's ammo related???

New GMR 13 14.5" Ready Rifle cleaned and lubed

Any Ideas what would cause this?

Thanks Guy's....I'll do a better job with my quality control procedures! 

FYI - Ammoload Headstamed brass was the culprit, stepped brass ( which was new to me, always learning).. Went  thru the ammobox found half dozen more Ammoload rounds pulled them all, everyone was a " stepped" case.

Additionally the rifle rocks...Threw a Razor Red Dot on it the day I got it, zeroed it with the headlights on my truck, went to a match the next day.. Shot a GM classifier with it then I started putting bad ammo in it:) This thing is going to be fun:)
